The Connecticut Athletic Trainers' Association (CATA) strives to improve the quality of health care for athletes, patients and clients and enhance the profession of Athletic Training, through leadership, education, and cooperative efforts with other organizations and allied health professions. The CATA is a 501(c)6 non-profit organization.

2012 Secondary School Athletic Trainer of the Year Nomination Form click here

DEADLINE FOR NOMINATIONS IS MAY 1, 2012 at MIDNIGHT, EST

Eligibility Criteria

An individual must
1.  Have a minimum of 5 years of experience with the secondary school setting in Connecticut
2.  Be a current member of the CATA
3.  Have a current Connecticut athletic trainer license
4.  Be in good standing with the Board of Certification, Inc.

Additional Criteria
1.  The candiate may not be nominated for the CATA Athletic Trainer of the Year Award during the same year
2.  An individual may only win this award once every ten years
3.  The President of the CATA is not eligible to win this award during the term of office

Award Criteria

1.  Individual must demonstrate exemplary commitment as an athletic trainer in the secondary school setting
2.  Individual must be a well-respected member of the school and the community
3.  Individual should be active in the advancement of the profession through such professional services, research, education, and public speaking

Nomination Process
Individuals nominated must submit a CV and the "Supplemental Form" to complete the nomination process
