The Connecticut Athletic Trainers’ Association awards two undergraduate scholarships, the District One Scholarship, given in conjunction with NATA District One, and the CATA undergraduate scholarship. The CATA also awards one graduate scholarship. The CATA offers these scholarships to athletic trainers who are enrolled full-time in a Connecticut college or university. Award recipients are chosen by the Research and Education Committee.
Penny F. Dunker-Polek Scholarship
Eligibility Requirements
1. Applicant must be a member of the NATA.
2. Applicant must be a full-time undergraduate student enrolled in an accredited Athletic Training
Education Program in Connecticut.
3. Applicant must have academic standing as a junior or senior (including 5th year)
4. Applicant must demonstrate an “above and beyond” attitude both as an athletic training student in the field and academically as well as other activities in which he or she may be involved.
5. Applicant must have a demonstrated commitment to the profession.
6. Applicant must be in good standing academically in accordance with the requirements of his/her Athletic Training Education Program. There is no other minimum GPA requirement for this scholarship.
7. Applicant must confirm his/her intent to pursue the athletic training profession as his/her
primary means of livelihood.
Nominating Instructions
The candidate must be nominated by a licensed athletic trainer in Connecticut. The nominating athletic trainer may not submit more than one nomination for this award.
The following items must be submitted:
1. A letter of nomination from a licensed athletic trainer, on business letterhead.
2. The student application form completed and signed by the nominee (pp. 3-4).
3. One additional letter of recommendation from a licensed athletic trainer, faculty member, or clinical
instructor on business letterhead.
4. Affidavit of enrollment completed by the Athletic Training Education Program Director (p. 5).
The deadline for filing applications for the 2010 CATA Penny F. Dunker-Polek Scholarship ONLY is May 1, 2010. All other CATA scholarship applications are due by April 1, 2010.

academic year.
6. Applicant must confirm his/her intent to pursue the athletic training profession as his/her primary
means of livelihood.

Each certified athletic trainer may submit no more than one nomination.
The following items must be submitted:
1. The student application form completed and signed by the nominee.
2. A letter of recommendation to be completed and signed by certified athletic trainer.

Eligibility Requirements
1. Applicant must be a full-time student attending a college or university in Connecticut.
2. Applicant must distinguish oneself academically, and as a participant in his/her athletic training
program.
3. Applicant must be, or plan to be, enrolled as a full-time graduate student in an athletic training
program.
4. Applicant may apply during his/her final academic year of undergraduate work, or anytime prior to
his/her final academic year.
4. Applicant must signify an intention to continue academic work toward a graduate degree as a
full-time graduate student.
5. Applicant must confirm his/her intent to pursue the athletic training profession as his/her primary
means of livelihood.
6. Applicant must be a member in good standing with the NATA.
